The purpose of the Children's Palliative Care Team is to support children, young people and their families along their palliative journey from diagnosis, to the approach to end of life and bereavement care. We take referrals from other professionals for palliative children and young people from diagnosis. We support professionals and teams around the child managing patients with palliative care needs in hospital and all community settings.

We support families along their palliative journey and develop advanced care plans centred around the child/young person/families wishes during life, symptom management, wishes at end of life. We lead end of life care for the palliative neonate/child/young person in the place of families' choice, in hospital, (Gully's Place) hospice or home
