  • A referral can be made by parents, social care or health care by emailing a completed referral form to the manager. Funding must be agreed prior to Shapes commencing service

Description:

Shapes Transition service offers support to young adults and their family to navigate through the change from child to adult services by providing a day service to meet complex needs.

Support provided -

  • For those with PMLD and complex medical needs, aged 18 – 25
  • Support term time, 9.30am – 3pm
  • Open 37 weeks of the year
  • Access to specialist activities and therapies
  • Highly trained staff including meeting medication and alternative feeding regimes
  • Fully adapted building in Poole with appropriate maintained equipment
  • Support offered with transition to Adult Services.
