Palliative Care - Children's Centre - Dorset County Hospital

Palliative Care services are provided and this is in partnership with Poole Hospital, the local hospices (Naomi House, Julia's House) and the palliative care team at Southampton hospital. Children and families are supported to develop an Advanced Care Plan for their child and at end of life are given choices regarding where they wish to be with an individual care plan which can include a 24/7 service at the end of life.
