Date(s) required, Child's Name, Age, School Year, School, Medical Conditions, Allergies, can we use photos of your child taken at the club on our website and any other information you feel we need to know. In addition, your name, mobile number and your email address.

Description:

LEVEL 2 - Your club is suitable for children who have mild to moderate needs, and those who will need direct support with personal care and will need one to one support with eating their lunch. This level is suitable for children who do not present risky behaviours such as being harmful to themselves or to others.
