We are committed to achieving the best outcomes for the young people in and leaving our care. We have a team of workers based in the team who specialise in preparing young people for independence and helping and equipping them afterwards.
When a young person becomes 16 the team help them to begin to plan for their future, including help with education, work, housing, health and money. This support continues until their 21st birthday or beyond if they are still in education at 21.
The transition to adulthood can be a very exciting but also very challenging time for young people and some things will change as you become an adult. For young people with additional needs and those currently receiving specialist services, we will support you to have a needs assessment from the Preparing for Adulthood Team if required, to see if you are entitled to any additional support from adult services as you turn 18.
