Preparing for adulthood (PfA) supports young people with special educational needs or disabilities (SEND) and their families, plan for the transition into becoming an adult.

TransitionTransitionTransition is the term used to describe the process of planning and preparation when children and young people change from one phase of education, health or social care to another. to health services should start at around 14 years of age. Access to the right information at the right time will help young people and families to plan and make choices that meet individual needs.
