Your health - preparing for adulthood

We want to help young people to understand why eating healthily, moving about or exercising and looking after themselves can help them to feel better and stay well.

The websites and help sheets in this section give lots of helpful advice and links to support young people to stay healthy. There is only 'one you' and we want you to feel great.

The LDR inclusion Tool – Carers guide explains why the learning disability register, annual health checks, and health action plans are important. It also explains how you can help make sure the people you support get the health services they are entitled to.

The publications section provides details about the importance of regularly checking your chest. Further information about health transitions and preparing for adulthood can be found in the  health section of the local offer.
