We want care leavers to reach their potential and be active members of society. We want you to have the same opportunities in life as other young adults. As young people in care approach adulthood they will be supported to think about and plan their future, such as, where they will live, what support they may need to find accommodation, employment and take part in their community. Most of this support will be provided by people already involved with them, such as a social worker, and the regular planning meetings that take place will have more of a focus on moving into adulthood and increased independence.
