Transition in education for young people with SEND aged 11 to 18 years

All students with special educational needs should be able to make a successful transition into adult life, including guidance on employment, independent living and playing an active part in their community.

Post-16 education providers have access to specialist skills to support the learning of all students with SEND. There should be a named person who is able to review the provision and make sure that any support is well co-ordinated and that teaching and support staff know who to contact if they need additional help with a student’s SENSENSpecial Educational Needs is a term which refers to children who have learning difficulties or disabilities that make it harder for them to learn than most children of the same age. or are concerned about the progress of a student.
